IMPORTANT ANNOUNCEMENTS

Bid Law Requirement Update: Act 406
Vendors will be required to submit certain documentation with SOQ submission, i.e. affidavits, proof of insurance, etc. These requirements will be outlined specifically in the instructions of each SOQ package. Vendors should continue to carefully read and respond accordingly per the requirements of the SOQ packages. These changes are pursuant Louisiana Public Bid Law more specifically, Act 406 of the 2016 Louisiana Legislative Regular Session. Occupational Medicine Services
The Parish of Jefferson, authorized by Resolution No. 142362, is hereby soliciting a Statement of Qualifications from licensed Occupational Medicine Providers interested in providing Occupational Medicine Services including Pre-Employment, fitness for duty and other physical examinations, specimen collection for drug and alcohol tests and administering medical treatment for Jefferson Parish employees and prospective employees.
Proposer must conduct business in an office/practice on either the East Bank or West Bank of Jefferson Parish. The Jefferson Parish Council may award these contract services to one or more vendors as they deem appropriate based on the submittals received.
Deadline for Submissions: 3:30 p.m., August 17, 2023
Minimum Qualifications
In order to be considered the proposer must meet the following:
1. The proposer must hold a current Louisiana State Medical License, and must be in good standing.
2. Proposer must conduct business in an office/practice on either the East Bank or West Bank of Jefferson Parish, or both.
3. Proposer’s office staff should consist of one (1) occupational doctor, at a minimum two (2) medical assistants, and one (1) support staff member.
Evaluation Criteria:
The following criteria shall be used to evaluate the Statements of Qualification the of vendors submitting.
1. Proposer’s qualifications. (25 points).
2. Demonstrated ability to respond to the Parish’s needs on a timely basis. The proposer’s office staff should at a minimum consist of one (1) doctor, two (2) medical assistants, and one (1) support staff member; please include names and titles of office staff and please state whether they have certifications and licenses that are related to occupational medicine; and identify any other relevant abilities of staff to respond. (25 points)
3. Demonstrated ability to provide required services; include professional experience in providing similar services to a population of 2,000 or more employees and include references from agencies for which similar scope of services are currently being provided which includes contact person and telephone number for each reference; identify the number of years that your practice has included occupational medicine, indicate whether it is full or part-time and other related experiences. (20 points)

5. Provide information demonstrating the Proposer’s understanding of the nature and scope of this project. (10 points)
Interested carriers must submit one (1) online submission.
The person or firm submitting a Statement of Qualification (General Professional Services Questionnaire) must identify all subcontractors who will assist in providing professional services for the project, in the
professional services questionnaire. Each subcontractor shall be required to submit a General Professional Services Questionnaire and all documents and information included in the questionnaire. (Refer to Jefferson Parish Code Ordinance, Section 2-928)
All persons or firms (including subcontractors) must submit a Statement of Qualifications (General Professional Services Questionnaire) by the deadline. The latest professional services questionnaire may be obtained by contacting the Purchasing Department at (504) 364-2678 or via the Jefferson Parish website at www.jeffparish.net. This questionnaire can be accessed by hovering over “Business and Development” on the website and clicking on the Professional Services Questionnaires option under “Doing Business in Jefferson Parish”.
Submissions will only be accepted electronically via Jefferson Parish’s e-Procurement site, Central Bidding at www.centralauctionhouse.com or www.jeffparishbids.net. Registration is required and free for Jefferson Parish vendors by accessing the following link: www.centralauctionhouse.com/registration.php.
No submittals will be accepted after the deadline.
Affidavits are not required to be submitted with the Statement of Qualifications, but shall be submitted prior to contract approval.
Disputes/protests relating to the decisions by the evaluation committee or by the Jefferson Parish Council shall be brought before the 24th Judicial Court.

Question: When the requirements state a Licensed Occupational medical provider is needed, is that qualified under the number of years of experience? Answer: The current La medical license in good standing is a minimum qualification. The years of experience in providing similar services is one of the evaluation criteria.

Question: Is there more detail on the component requirements for the new hires testing. What physicals and drug testing is ais needed specifically? Answer: For Pre-Employment physicals, it would be a base evaluation based on the physical, environmental and sensory requirements of the job.. That information will be provided.. For example, if it is a CDL position- we would need a DOT physical. The drug testing is to screen for detectable substances as contained in our policies for random pre-hire, post-accident and reasonable suspicion test.

Question: Does this also include injury treatment for employees? Answer: Yes Question: Also wanted to verify the physicals that are required. I know we saw Pre employment physical and fitness for duty. Can you verify if this is a Basic preplacement physical or DOT, and if what level the fitness for duty is required (Levels 1-5) Answer: For pre-employment, yes physicals are required. Fitness for duty physicals are for current employees that may have been injured on the job or reported to work with some sort of injury/medical condition. The physical will be based on the needs of the job, there may be a need for a DOT physical at times, bit this would be noted when the employee is sent. As far as for the level of the fitness for duty, we would provide the clinic with the ergonomic conditions and essential functions of the job, and the physical would determine if the employees is fit to perform those functions. If they are not, the physical would state what limitations/accommodations that they would need. Question: As far as onsite/weekend hours that are requested, what services are needed, what would the hours needed as well as the location for this service? Answer: Afterhours: This would be for drug testing due to an accident or injury afterhours; in that we would call the provider to arrange a location/time for them to meet with the supervisor to undergo a drug screen. For onsite: we would like the provider to be able to provide random test collection services at least 1 day a week in her office. We have the space set up to provide the testing and would need a collector and he/she would send the samples off to the lab to be tested.

Question: Do we need to provide resumes & CV's for all center colleagues or just the medical support staff? Answer: You can provide the information for those employees that would work on the JP contract if it was awarded to them.
