REQUEST FOR PROPOSAL

The Parish of Ascension hereby issues a Request for Proposal (RFP) for Emergency Debris Removal and related Services.  Request for proposals will be received by Ascension Parish Purchasing Department, 615 East Worthey Road, Gonzales, Louisiana 70737 (P.O. Box 2392, Gonzales, Louisiana 70707) until SEPTEMBER 17, 2020 at 04:00 p.m. (CST) for the following:

Scope
This statement of work describes and defines the services, which are required for the execution of Natural Disaster-related emergency debris removal from local and private roadways within the Parish of Ascension.  The contractor shall provide all services described herein and any other services required to complete the project.  Activities include, but are not limited to, field operations, debris pickup, debris hauling and removing, debris staging and reduction, temporary debris storage site management and debris management.                                                                                                                                                                                                                                                   
 
 
Specifications may be obtained at the Ascension Parish Government, Purchasing Department located at 615 E. Worthey Street, Gonzales, Louisiana 70737. Qualifications, proposals, and bids can be submitted on-line at http://www.centralauctionhouse.com. Specifications and support documents can also be viewed and downloaded at http://www.centralauctionhouse.com.    The Ascension Parish Government shall not be responsible if the bidder cannot complete and submit an RFP due to failure or incomplete delivery of the files submitted via the internet.
 
The Parish of Ascension reserves the right to disqualify any Bid, response to a Request for Qualifications, or Request for Proposals if it is determined that the submitting business entity is not in good standing with the Louisiana Secretary of State or is not authorized to do business in the State of Louisiana.
 
Ascension Parish Government reserves the right to reject any and all bids for just cause.

Typically RFP's of this sort or awarded based on a % of other variables accompanied with a proposal. Reading this document does the parish plan to award this strictly based off of price?

Addendum 1 posted a revised pricing sheet which changed incineration to compaction for C&D reduction. It also omitted the last page of pricing which included the price total and line items for E-waste, Animals Carcasses, and Tire Removal. Please confirm if the addendum replaces the entirety of the Exhibit C Fee Rates form, or just the first page.

Is Standard Form APG-1001 required to be filled out and submitted with our proposal? Or is this a post award document?

Standard Form APG-1001 states that "All subconsultants/associates listed for this project must attach a signed Form APG-1001" but the first page states that it can only be signed by the official with the signing authority for this contract. Please confirm if subcontractors are to fill out this form in it's entirety with the proposal or if they are to merely sign p.8 as an acknowledgment.
