Question 1: Why has The City decided there is a need for a bid at this time?
Question 2: Please provide a copy of the current agent of record contract, along with the current fee. Please verify if the current broker receives any commissions or subsidies over and above the stated fee.
Question 3: Please include the current Scope of Work if different than the one listed in the RFP.
Question 4: Please confirm whether the medical and dental plans are self-insured or fully insured.
Question 5: Please provide a breakdown that provides the number of employees on medical, dental, vision, and any ancillary products by tier levels (i.e. EE only, EE and Spouse, EE and Child, and EE and Family),
along with monthly premiums and any ER and EE premium splits.
Question 6: Copy of Pharmacy contract and pharmacy claims data for the most recent 2 years, if available.
Question 7: Medical claims data for the most recent 2 years, if available.
Question 8: If available, the number of employees that have exceeded the out-of- pocket maximum over the past two or three years.
Question 9: Does the deductible, co-pay, and/or out-of-pocket maximum cause an undue burden on employees and their family under the current plans?
Question 10: What are the current steps for the benefit premium collections, reconciliation, and remittance to the carriers?
Question 11: Does the current broker provide any other Benefit administrative services without the support of a third-party administrator? (i.e. billing reconciliation, deduction file audits, eligibility file audits, 1094 or 1095
filings).
Question 12: On page 4, under the Renewal section, The City has for “benchmark pricing”. Please provide a copy of the most recent pricing report completed by the current broker. If not available, please specify the
report headings that The City expects to have provide in the benchmark report.
Question 13: What payroll system does the city utilize?
Question 14: How does The City administer open enrollment, new hires, terminations, and change in status (CIS) events and what role does the current broker play in assisting The City to manage these items?
Question 15: For open enrollment, does The City offer face-to-face, call center, or self- enrollments? Or a combination of the 3 options?
Question 16: What are The City’s normal dates for open enrollment?
Question 17: How does The City communicate the open enrollment messages (e-mail, text, written letters, postcards, etc.)?
Question 18: Does The City or Broker produce an Employee Benefit Guide? If yes, what entity produces the guide and is it a printed version or included on The City’s website? May a copy of the guide (or link) be
provided if available?
Question 19: Does the City offer a standalone EAP program and if so, which EAP vendor is in place?
Question 20: How does The City administer retiree benefits, i.e., internal or external administration? If internal, would The City consider allowing both employees and retirees to be combined under the same
administrator for monthly billing and reconciliation?
Question 21: What benefits are retirees allowed to enroll in other than medical services?
Question 22: Please explain how retirees are processed when moving from an active status to a retiree status.
Question 23: How does The City monitor and transition retirees from The City’s coverage to Medicare once the retiree’s approach 65?

Question 34: Does the City currently utilize an actuary for any services related to the medical plan? If so, which firm is utilized?
Question 35: How does the city currently perform its ACA 1095-C/1094-C reporting requirements? Which vendor does the city utilize?
Question 36: Who prepares The City’s section 125, cafeteria plans?
Question 37: Does the City allow any part-time employees to receive benefits? If yes, please describe the benefits available.

Question 39: What role does the current broker play in the City’s wellness efforts?
