PUBLIC NOTICE-INVITATION FOR PROFESSIONAL SERVICES 
The Lafayette Parish School Board (LPSB) is seeking Statements of Qualifications (SOQ) from highly qualified firms interested in providing complete Professional Services for the following projects:
 
Professional Services - Feasibility Study
Renovations or Replacement of
J.W. Faulk Elementary School
RFQ# 30-21 
Statements of Qualifications shall only be submitted on Lafayette Parish School Board Form LPSB-SOQ (01/08/2021). A sample form is attached and a computer file can be provided upon request.
 
Interested firms may obtain an official Request for Qualifications (RFQ) package from the LPSB Purchasing Department or by visiting the Purchasing Department website at https://www.lpssonline.com/site6424.php.  Qualifications information may also be viewed online, and electronic Statements of Qualifications may be submitted online at www.centralauctionhouse.com.
 
Only those firms that have obtained the official RFQ package for this project from the LPSB Purchasing Department, LPSB Website, or Central Auction House will be considered by the LPSB-SOQ Evaluation/Selection Committee. 
The original of the Statement of Qualifications (Form LPSB-SOQ 01/08/2021) shall be delivered to Purchasing Agent, Attn: Mr. Lee Francis, Lafayette Parish School Board, 113 Chaplin Drive, Lafayette, LA 70508. Five (5) additional copies of the Statement of Qualifications Form LPSB-SOQ (01/08/2021) may be included in the submittal; however, one form LPSB-SOQ (01/08/2021) is to be clearly marked as “Original”. Please note that Prime consultants must use the Standard Form LPSB-SOQ Prime (12/22/2020). Only Prime Consultant forms will be required for submission. Statements of Qualifications for this project will be accepted until 2:00 P.M., Central Standard Time, 02/17/2021.
 
Statements of Qualifications that have not been received by the above aforementioned deadline date and time will be rejected. Additionally, failure to submit all of the information on Standard Form LPSB-SOQ (01/08/2021) shall be considered non-responsive and may result in the Qualifications Statement being rejected.

Additionally, the members of the LPSB SOQ Selection Committee request that the consultants submit Form LPSB-SOQ (01/08/2021) only. Unless otherwise stated or required by the instructions, all other attachments or embellishments shall be excluded. It is also requested that Form LPSB-SOQ 01/08/2021) be stapled rather than being bound in a booklet.
 
Proposal Respondents, their consultants, sub-consultants, or other parties representing the proposer for this solicitation may not contact any LPSB Board Member, the LPSB Superintendent, and/or any member of the Evaluation/Selection Committee concerning this project from the date of the solicitation until the date of selection.
 
The Lafayette Parish School Board reserves the right to reject all of the submittals in response to this Request for Qualifications.
 
The Lafayette Parish School Board reserves the right to cancel this Request for Qualifications at any time.
