Jefferson Parish Department of Purchasing is soliciting Request for Proposals (RFP’S) from qualified firms to provide a Wellness/Fitness Program for the Jefferson Parish East Bank Consolidated Fire Department. 
 
The Jefferson Parish “East Bank Consolidated” Fire Department (J.P.F.D.) is seeking a two (2) year contract with a medically-supervised physical fitness program vendor with an on-staff physician to manage its existing Wellness Program.  This program is a comprehensive wellness initiative requiring that the vendor shall provide initial physical examinations/assessments of all personnel by a licensed physician, annual fitness testing/assessments, and a structured fitness program tailored to each employee’s individual needs.  The total expected number of mandatory participants is 238.  
 
All proposals will be evaluated on criteria such as vendor’s technical proposal, qualifications and experience, financial profile and proposal responsiveness and other criteria more specifically defined in the RFP document.  The maximum total points for each proposal are set at 100 points
