Bid Law Requirement Update: Act 406
Vendors will be required to submit certain documentation with SOQ submission, i.e. affidavits, proof of insurance, etc. These requirements will be outlined specifically in the instructions of each SOQ package. Vendors should continue to carefully read and respond accordingly per the requirements of the SOQ packages. These changes are pursuant Louisiana Public Bid Law more specifically, Act 406 of the 2016 Louisiana Legislative Regular Session. SOQ 21-011
 
            The Jefferson Parish Council is hereby soliciting Statements of Qualifications from persons or firms interested in providing Health and Dependent Care Flexible Spending Account administration for active employees and their dependents. The enrollment period is approximately mid-October through mid-November 2021, to be effective on
January 1, 2022.

The person or firm submitting a Statement of Qualifications (Jefferson Parish Professional Service Questionnaire) must identify all sub-contractor firms which they expect to use to provide professional services, and submit a Statement of Qualifications (Jefferson Parish Professional Services Questionnaire) for each sub-contractor firm. (Refer to Jefferson Parish Code of Ordinance, Section 2-928).
 
MINIMUM QUALIFICATIONS

Proposer must be licensed in Louisiana and in other states once it is known that a beneficiary has moved to or received services in that state.  The proposer should also indicate in their response how they are in compliance and have a working knowledge and understanding of the claims payment laws and other laws in accordance with Section 125 of the Internal Revenue Code.  Please provide copies of all licensing credentials from the State of Louisiana with your proposal.
Proposer must have at least five (5) years of experience in providing the type of plans and services requested in this SOQ.
Proposer must offer the type of plans and services as described in this SOQ to at least three (3) similar employer groups or municipalities with similar total members as Jefferson Parish Government and provide a list of references. 
EVALUATION CRITERIA: The following criteria will be used to evaluate each firm submitting a Statement of Qualifications:

Demonstrated Experience- providing quality FSA/DCA Administration for active employees and dependents for large groups (3000+ members)– 25 Points Maximum
Financial Proposals- Rates for plan chosen – 30 Points Maximum
Company’s Financial Strength – based on A.M Best Rating or Standard and Poor’s (a rating of B or below will receive 0 points)- 20 Points Maximum
Demonstrated ability to provide customer service and  plan administration, and member service. – 25 Points Maximum 
COMPENSATIONCompensation for the required services will be made in the form of monthly payments from the accounting department.
 
 
All firm(s) must submit a Statement of Qualifications; a Jefferson Parish Professional Service Questionnaire; if using a sub-contractor, a Jefferson Parish Professional Service Questionnaire the proposed rate form, a signed scope of services, a completed carrier questionnaire, a signed addendum along with any other information requested in the SOQ.

Affidavits are not required to be submitted with the Statement of Qualifications but shall be submitted prior to contract approval, including notarized affidavits for Sub-Contractors.
 
Disputes/Protests relating to this procurement shall be brought before the 24th Judicial District Court per Jefferson Parish Code of Ordinance § 2-932.
